Commotio cordis due to high-velocity projectile ejected from an industrial lawnmower

چکیده
Introduction Commotio cordis is a term used to describe ventricular fibrillation (VF) triggered by a blunt, nonpenetrating blow, often the result of a small ball or puck hitting the chest in the course of recreational or competitive sports. It is typically fatal, though rates of successful resuscitation have improved in recent years. Experimental studies have identified a 10– 40-ms window during the upstroke of the T wave during which a blow of sufficient force can elicit VF, with 40 mph being the optimal projectile velocity for inducing VF in a swine model. Children engaged in sports with the potential for high-impact blows are most at risk.

Clinical profile and spectrum of commotio cordis.
CONTEXT Although blunt, nonpenetrating chest blows causing sudden cardiac death (commotio cordis) are often associated with competitive sports, dangers implicit in such blows can extend into many other life activities. OBJECTIVE To describe the comprehensive spectrum of commotio cordis events. DESIGN AND SETTING Analysis of confirmed cases from the general community assembled in the US Comm...
متن کاملComplete heart block following a blow on the chest by a soccer ball: a rare manifestation of commotio cordis.
Commotio cordis is a rare type of blunt cardiac injury in which low impact chest trauma causes sudden cardiac arrest, usually occurs from being struck by a projectile during sports. The most common arrhythmia during commotio cordis is ventricular fibrillation, although complete heart block and an idioventricular rhythm have also been reported. We describe a case of a young patient who presented...

متن کاملUpper and lower limits of vulnerability to sudden arrhythmic death with chest-wall impact (commotio cordis).
OBJECTIVES In an animal model of commotio cordis, sudden death with chest-wall impact, we sought to systematically evaluate the importance of impact velocity in the generation of ventricular fibrillation (VF) with baseball chest-wall impact. BACKGROUND Sudden cardiac death can occur with chest-wall blows in recreational and competitive sports (commotio cordis). Analyses of clinical events sug...
متن کاملArrhythmia/Electrophysiology Marked Variability in Susceptibility to Ventricular Fibrillation in an Experimental Commotio Cordis Model
Background—Precordial blows in sports and daily activities can trigger ventricular fibrillation (VF) (commotio cordis). Whereas chest wall blows are common, commotio cordis is rare. Although factors such as timing, location, orientation, and energy of impact are critically important, we also hypothesize that there is individual susceptibility to commotio cordis. Using our model of commotio cord...
